COVID Travel Pass
If you require proof of vaccination or recovery from COVID this needs to be obtained via the NHS App or from 119. Stockbridge Surgery is unable to provide letters for patients regarding vaccination status or proof of recovery.

Do you need vaccinations?
Our Nurses can provide comprehensive travel health advice and arrange for appropriate immunisations. Please complete a travel assessment form (available below) for each traveller and ensure it is returned to the surgery at least 8 weeks before your date of travel. We prefer to receive forms electronically. Vaccines need time to take effect and some may require a course over several weeks.
You can find out which vaccines you may need and other health advice by choosing the country / countries you are travelling to on this website: NaTHNaC - Country List Your vaccination history can be found on the NHS App or you can contact the surgery by emailing us on hiowicb-hsi.stockbridgesurgery@nhs.net to request a copy.
Our nurses will still require you to complete a travel assessment form even if you only require vaccinations which are free on the NHS.
Travel Assessment
We offer a full travel advice and immunisation service. If you are travelling abroad please complete and submit the travel form in plenty of time (at least 8 weeks) before your journey.

Travel Vaccination Prices
Travel advice is covered by the NHS; however some vaccinations and prescriptions are only available privately. Payment for non-NHS services must be made before any appointment is booked.
