• When a patient is discharged from hospital and requires support or care at home, this should be arranged before they are discharged.  
• Long term care will be discussed ahead of discharge, and the hospital will arrange referral to the local authority for support or to a nursing home. 
• Stockbridge Surgery receive a copy of your discharge summary, this will go to the doctor to action. The hospital supply 14 days of medication so this gives the GP time to add your new medication to your records or make any changes.
• If you’re having a planned procedure privately and require help or equipment on discharge, this needs to be arranged ahead of your procedure.

If you think your loved one cannot cope at home following their hospital discharge please inform hospital staff so that care arrangements can be made.
