Community nurses provide nursing care, treatment and support to elderly, housebound or chronically ill patients, or those who have recently had an operation. These include special interests such as leg ulcer management, continence management, blood tests and palliative care.  

This service is only for those who are physically unable to get to the surgery and the Community Nurses do not make specific appointments. Referrals can only be made by GPs and hospital staff.
