MEDICAL EXAMINATION & MEDICAL REPORT's:

• Vocational driving HGV/PSV Exam  (Please note the eye sight section of these forms should be completed by a registered optician)
• Taxi Driver/other occupational driving medical (Please note the eye sight section of these forms should be completed by a registered optician)
• Solicitors Reports

INSURANCE FORMS:

• Travel insurance cancellation certificate 
• Private Health Insurance Claim certificate
• School Fees Insurance Claim certificate 
• Sickness/accident insurance claim certificate

MISCELLANEOUS/OTHER FORMS:

• To Whom It May Concern Letter
• Firearms Licencing
• Private sick note

There are certain non NHS reports that we do not offer in line with advice from the RCGP and BMA. This is because they may require specialist knowledge:

Examples include:

• “Fitness to" run a marathon/sporting event/gym
• Scuba Diving Medicals

The doctors reserve the right not to undertake any private work that they feel is outside of their scope of competence.

If you are unsure please hand your forms into reception and one of the doctors will then decide if they can undertake the work.
